What do I need in order to complete enrollment with Stax?

Having the following details on hand will make the enrollment form quick and easy:

 

Organization Information

  • Legal name
  • EIN / Tax ID
  • Contact information
  • Incorporation date

Your Information

  • Contact information (name, address, phone, email)
  • Date of birth (DOB)
  • Social Security Number (SSN)
    • Note: Your SSN is required as part of federal regulations, including the Patriot Act.

Bank Account Information (for the business' checking account; cannot be a savings account)

    • Business Name on Bank Account
    • Routing Number
    • Account Number

Documents

  • If you are a registered 501(c)(3), you'll need your 501(c)(3) Determination Letter that you received from the IRS.
    • You can request copies by submitting this form to the IRS or searching for your organization with this IRS search tool (if issued January 1, 2014 or later).
  • If you currently sell or plan to sell prescription medications through Shelterpay, you'll need to submit a colored copy of the front and back of your driver's license.
