Information Needed to Complete Your Enrollment with Stax

Before you start your enrollment with Stax, please make sure you have the following information on hand:

Organization Information

  • Legal name
  • EIN / Tax ID
  • Contact information
  • Incorporation date

Your Information

  • Contact information (name, address, phone, email)
  • Date of birth (DOB)
  • Social Security Number (SSN)

Your SSN is required as part of federal regulations, including the Patriot Act, even if your organization is a registered non-profit.

Business's Bank Account Information

  • Business Name on Bank Account
  • Routing Number
  • Account Number

You are required to use a checking account. Savings accounts will not work.

Documents

  • If you are a registered 501(c)(3), you will need your 501(c)(3) Determination Letter that you received from the IRS.

    You can request copies of the letter by submitting this form to the IRS or searching for your organization with this IRS search tool (if issued January 1, 2014, or later).

    If you are not a registered 501(c)(3), you can still enroll in Shelterpay. You will simply select the appropriate business entity type on the enrollment form (such as LLC, corporation, etc.).

  • If you currently sell or plan to sell prescription medications through Shelterpay, you will need to submit a colored copy of the front and back of your driver's license.
