Instead of going through each medical tab on an animal record when you need to quickly add multiple things like a microchip, treatments, and vaccines, you can create a custom medical form where all the fields can be seen on a single form.

This is useful for common tasks, such as intake exams or treating common diseases, and will save you tons of time with manual data entry. To learn more about using a custom medical form on an animal record, take a look at How to Use a Custom Medical Form.

Click your Username > Configuration > Medical > Custom Medical Forms to get started.

What user permissions do I need to configure a custom medical form?

Note
Configuration is only accessible through a desktop or laptop. You cannot access the Configuration section on a mobile device.

How to Create a New Custom Medical Form

Follow these steps to create a custom medical form:

• From the Custom Medical Form configuration section, click the green Add Custom Medical Form button.
• Give your new medical form a name and click the green Save button.
• You will now see the sections listed below that you can open and configure. Each section will have a list of fields you can toggle visible and required.

You can quickly expand each section by clicking the Expand All button on the right-hand side of the screen. You can also reorder fields how you see fit using the double line on the left-hand side of the field name.

How to Edit or Delete a Custom Medical Form

• Follow these steps to edit a custom medical form:

  • Click the Type dropdown menu and select the appropriate form.

  • Open each section and adjust as needed.

  

• Follow these steps to delete a custom medical form:

  • Click the Type dropdown menu and select the appropriate form.

  • Click the trashcan next to the form's name.

  • Read the warning and click the red Delete button to delete the custom medical form.

  You cannot recover custom medical forms that you delete.

Example Custom Medical Forms

• This form is used after a kitten is intaked and gets its first exam. In this example shelter, kittens get a brief physical exam by a tech, FVRCP, FeLV tested, dewormed, flea treatment, and might get put on the Vet Checklist if there is a problem. Here are the fields that the shelter chose:
