After you have set up your custom medical forms in configuration, you can add a custom form to an animal by one of three routes:

1. After Intake

After creating an animal record you can go straight into a custom form by click on "Start Intake exam".

2. Edit Multiple

From the home page you can select one or more animals and then choose a custom form from the dropdown. Some fields are not compatible with edit multiple, e.g. dog-only vaccine templates won't show up if you select a dog and a cat.

3. Tasks

When completing a Vet or CVT check you can do a regular physical exam or use a custom form by clicking on the complete button.
