Setting up Custom Medical Forms

When you want to quickly add many things to an animal, e.g. add a microchip, treatments, and vaccines, instead of going through each tab in the animal record or doing each step in Edit Multiple you can create a custom medical form where all the fields can be seen in a single screen. This is most useful for commonly used tasks such as intake exams or treatment of common diseases.

To create a custom form, open Configuration > Medical > Custom Medical Forms and click Add custom form. In this window, pick a name such as Kitten Intake.

You can then choose which fields you want to show up in your form. You can pick from the commonly used animal and medical fields (e.g. vaccines, memos, health checks) To turn a field on, click on Show Options and click on the eye icon. All fields are turned off by default. Clicking on the asterisk icon means that the field will be required in order to save. You can also drag and drop fields to change their order. Choose the fields that are applicable for form and then click save. Once created, your form can be edited in configuration.

Example Custom Medical Forms

  • This form is used after a kitten is intaked and is getting its first exam. In this example shelter, kittens get a brief PE by a tech, FVRCP, FeLV tested, dewormed, flea treatment, and might get put on the Vet Check list if there is a problem. Here are the fields that the shelter chose:

  • This form is used for rechecking dogs undergoing URI treatment. Animals receive a brief recheck, may have more treatments prescribed, a diagnosis added/resolved, a memo added, and/or a health check added. Here are the fields that the shelter chose:

    If you choose to show the Physical Exam template drop down it will only fill out fields you have chosen to show in the custom form.
  • This form is used for animals who test positive for heartworms who undergo pre/post-injection treatments and to schedule follow-ups:
